If you do this before your session, it will save time during the session. However, if you prefer to complete the forms in your session,
Alison will provide you with the relevant paper forms on arrival.
The key things you can do before your session are:

- 2. Download, read and sign the consent form found here.
- 4. Complete the online version of the Depression, Anxiety and Stress Scale (DASS-21) here
- 5. Download the address and directions information found here
- 6. Read policies on your rights and privacy found here and here.
- 5. Remember to take along your GP Mental Health Care Plan (if relevant).

The consent form must be signed prior to starting your first session to indicate that you consent to the conditions of service. You can
save time in your first session by reading, printing and completing the form prior to the session and bringing it to the session with you.
You will be given an opportunity to ask any questions prior to signing the form. Otherwise, it will be made available to you in your first session.

Registration form and DASS measure
It will also save you session time if you provide your contact details and some other relevant information
prior to attending using the online form. This is stored on a secure site. If you do not feel comfortable providing these details prior to the session,
these details can be provided in person, during the session. Alternatively, you can complete a paper copy of the form and bring it with you.

Lantern Psychology is part of a multidisciplinary team at Capital Clinical Psychology, so once you will need to look out for signs to Capital Clinical Psychology.
We are located on the lower ground floor of the building on the corner of King and Denison Streets,
labelled the 'Deakin Sports Therapy Centre'. Enter the building from King Street (not Denison Street)
at Entrance B (there is a sign). Walk down the steps or ramp from King Street, once inside, turn right
and walk through the interior doors, then turn left up the corridor.
